Providence Home Health Care can provide you and your loved one(s) with many services:

Skilled Nursing
Registered Nurses (RN’s) are involved in creating a care plan for the patient with the input of the physicians and family to ensure that all needed aspects of the patients care are actively being addressed. In addition, the RN will continually monitor your progress and adjust your care plan as needed.

Skilled nursing staff will recommend which services would be beneficial to the patient based on the care plan. This plan may include some or all of the following services:

  • Diabetic care, management, and education
  • Post surgical recovery
  • Ongoing wound care
  • Cardiac care
  • Medication management


Behavioral Health
A Behavioral Health Nurse cares for patients who are acutely unwell or have an enduring mental health problem.

The behavioral health nurse can provide any or all of the following services:

  • Assess signs and symptoms of psychological needs
  • Formulate individual care plan
  • Provide supportive therapy for dementia, depression, anxiety, etc
  • Provide ongoing education regarding behavioral issues
  • Coordinate other resources
  • Provide skilled intervention when needed
  • Monitor medication compliance

 

Physical Therapy
Physical Therapy is a form of treatment consisting of exercising specific parts of the body such as the back, legs, arms, hands or neck. Physical therapists evaluate the patient’s level of functioning and help to improve their function.

The patient’s goals may include:

  • Increase in strength
  • Increase in range of motion
  • Partial or complete rehabilitation of the musculoskeletal system to improve function 

 

Home Health Aid Services
Under the direction of an RN or Physical Therapist, our highly trained Home Health Aides will follow the patient’s individualized care plan by providing any or all of the following services:

  • Prepare a special diet and assist with feeding
  • Monitor ongoing conditions
  • Assist with daily exercise and rehabilitation activities
  • Identify and report symptoms requiring attention
  • Assist with bathing, dressing, and hair care
  • Assist with toileting including caring for a catheter or ostomy bag

 

Occupational Therapy
Occupational therapists work with the disabled, the elderly, newborns, school-aged children, and anyone who has a permanent or temporary impairment in their physical or mental functioning. Occupational therapy can retrain and restore an individual’s functioning to individual optimum level. The purpose is to help patients perform daily tasks in their living and working environments.

 

Do I need Home Health Care?
Your physician may recommend additional home health care services. Additionally, you or a loved one may decide that extra help with daily personal care, for example, would be beneficial to you for a period of time.

  • Have questions about medications, even after the doctor’s visit
